29 CFR  1926.1051(a):  Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:   On or about April 16, 2025, an employee was exposed to fall hazards of greater than 8 feet, while accessing and exiting the top-plate via a 6-foot stepladder that was more than 19 inches below the top-plate, on a residential construction site in Normal, Illinois.
