29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. a) On or about April 7, 2025, on the east side of the drawbridge # 117: subcontractor employees were exposed to a 12 foot fall hazard while working on the bridge, in that the employer did not ensure that each employee was protected from falling through an access panel leading to the mechanical room.
