29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) (a) At the I-95 South bridge over the Ogeechee River in Richmond Hill, GA - On or about April 5, 2025, and at times prior to, subcontracted employees were exposed to fall and drowning hazards, in that, the employer did not conduct frequent and regular inspections of the job site to ensure it was free from recognized hazards while employees were conducting work over water.

29 CFR 1926.106(d): A lifesaving skiff was not immediately available at locations where employees were working over or adjacent to water (a) At the I-95 South bridge over the Ogeechee River in Richmond Hill, GA - On or about April 5, 2025, and at times prior to, the employer exposed subcontractor employees to a drowning hazard, in that, the employer did not have a lifesaving skiff immediately available for subcontractor employees that worked from a suspended chain link scaffold over the Ogeechee River.
