29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Building roof - On or about April 14, 2025 and at times prior thereto, employees were replacing a roof on a 4 story building without the use of fall protection. Employer did not ensure the employees wearing harnesses, were connected to the lifelines with lanyards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) 80 Jefferson St. Troy, NY - On or about April 14, 2025 and at times prior thereto, employees were exposed to fall hazards of approximately 50' and had not been trained by the employer to recognize and minimize such hazards.
