29 CFR  1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  Location: Worksite  On or about April 17, 2025, employees were exposed to fall hazards over 20 feet while performing roofing activities, where the employer did not ensure that a competent person made frequent and regular inspections of the site.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  Location: Worksite  a) Fall hazards of over 20 feet while performing roofing activities. b) Fall hazard due to an open hole where the skylight had not yet been installed.  On or about April 17, 2025, employees were exposed to fall hazards of over 20 feet while performing roofing activities, where the employer did not ensure that proper fall protection was in place.
