Employees were not protected by guardrail systems, safety net systems, or a personal fall arrest system.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support): On or about April 17, 2025, at a jobsite located at 1204 Tallmadge Rd, Kent, OH, an extension ladder with a working length of approximately 10 feet, which was placed such that the foot of the ladder was approximately 4.35 feet away from the top support, resulting in an angle of 64.16 degrees.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: On or about April 17, 2025, at a job site located at 1204 Tallmadge Rd., Kent OH, an extension ladder, which was being used to access the lower roof of a building, was extended over a pedestrian walkway which was in close proximity to a busy road. There were no cones or caution tape being used to bring visual awareness to the ladder placement.
