Ogden Avenue, Naperville, IL 60563. On or about April 17, 2025, the employer did not ensure a non-self supporting straight ladder was extended 3 feet (.9 m) above the upper landing of an excavation, thereby exposing employees to falls of 9.4 feet (2.8 m). Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about April 17, 2025, the employer did not ensure that an extension ladder with a broken and bent rung was removed from service. Employees using it to access an excavation were thereby exposed to falls of 9.4 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
