29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  a) On or about April 17, 2025, at 1303 Utah Blvd Orlando Florida 32803: employees were exposed to overhead struck by hazards while employees where cutting engineer trusses, laying plywood and working with tools while employees are working below without protective helmets.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)  a) On or about April 17 2025, at 1303 Utah Blvd Orlando Florida 32803; Employees were exposed to fall hazards ranging from 10 -22 feet  while installing framing material on  a residential two story roof, with a pitch of 5/12 without the use of fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a) On or about  April 17 2025, at 1303 Utah Blvd Orlando Florida 32803, employees were using a Gorilla  extension ladder to access the roof, and the side rails were below the roof landing, exposing employees to fall hazards of approximately 10 feet.
