Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  (a) Commercial building rooftop, various locations; On or about April 16, 2025, employees were not protected from falls, while engaged in roofing replacement activities from a 4:12 pitch roof and near edges, that ranged approximately 11-feet, 9-inches to 12-feet, 10-inches above ground level.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   (a) Commercial building rooftop, various locations; On or about April 16, 2025, a Gorilla multi-position aluminum ladder (model MPX17), configured as an extension ladder, was used by employees to access and egress the rooftop. The ladder side rails did not extend at least 3-feet above the landing surface.
