29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On April 17, 2025, at the above addressed jobsite, employees were exposed to struck-by hazard from falling tools, material and debris when working below employees performing masonry work.  The employees were not wearing hard hats.  No abatement certification or documentation is required for this item.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.     a)  On April 17, 2025, at the above addressed jobsite, employees were exposed to fall hazards while accessing and egressing the work platform of a frame scaffold when using erecting ladders for climbing.  No abatement certification or documentation is required for this item.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On April 17, 2025, at the above addressed jobsite, employees conducting masonry work from a frame scaffold, 11 feet above the ground, were not provided with guardrails, midrails, cross rails or other means of fall protection, thereby exposing the employees to falls.  No abatement certification or documentation is required for this item.
