29 CFR 1910.178(q)(1):Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: a) Facility: On or about April 17, 2025, the employer exposed employees to a potential crush-by hazard, in that a forklift was not taken out of service that had damaged outer insulation that exposed the inner insulated conductor and amber warning lights that did not work.
