29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29CFR 1910.147(c)(4)(ii)(A), 29CFR 1910.147(c)(4)(ii)(B), and 29CFR 1910.147(c)(4)(ii)(C). On or about April 14, 2025, the employer did not ensure that the hazardous energy control procedure for the #9 Banbury Mixer F Series, Serial number 13557 in the western corner of the building on the 2nd floor, included a specific statement of the intended use of the procedure [per 1910.147(c)(4)(ii)(A)] or specific procedural steps for shutting down the equipment [per 29CFR 1910.147(c)(4)(ii)(B)] or the specific procedural steps for the removal of lockout devices [per 1910.147(c)(4)(ii)(C)].

29 CFR 1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about April 14, 2025, the employer did not ensure the periodic inspection of the energy control procedure for the Banbury Mixer #9, F Series, Serial number 13557, in the Western corner of the building on the 2nd floor, was conducted to correct any inadequacies in the procedure. The employer's energy control procedure was inadequate in that the employer did not ensure that the hazardous energy control procedure included a specific statement of the intended use of the procedure [per 1910.147(c)(4)(ii)(A)] or specific procedural steps for shutting down the equipment [per 29CFR 1910.147(c)(4)(ii)(B)] or the specific procedural steps for the removal of lockout devices [per 1910.147(c)(4)(ii)(C)].
