29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) At the site on 2855 Brooks St, Lakeland, Florida in the fabrication area on or about 4/17/25, the employer exposed employees to fall hazards, in that, employees were not protected from falling from the walking working  surface on top of the Generator roof approx. 10'9" feet in height while conducting drilling and tapping to topside.

29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a)  At the site on 2855 Brooks St, Lakeland, Florida in the fabrication area  on or about 4/17/25, the employer exposed employees to struck-by/crush-by hazards, in that, employees who operated powered industrial trucks on site had not been trained on the type of powered industrial trucks that they operated.

29 CFR  1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized:  a) At the site on 2855 Brooks St, Lakeland, Florida in the fabrication area  on or about 4/17/25, employer exposed employees to an electrical shock hazard, in that, the ProGen Tagout procedure for electrical was not complied with, in that an electrical cable was coiled and a red tape with the words "Do not use"  wrapped around it placed on an electrical switch box.
