29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards:  a) In front of 2705 Rosemont Ave., Wilmington, DE 19802: On or about April 18, 2025,  at the point of hazard, employees were exposed to struck by hazards when the employer failed to provide signs visible to the public while traveling on Rosemont Ave., South to assist traffic in understanding traffic pattern changes due to water main installation work. Employees were exposed to multiple hazards onsite including but not limited to struck by/ caught in hazards when they performed water main installation work.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) In front of 2705 Rosemont Ave., Wilmington, DE 19802: On or about April 18, 2025, employees were exposed to caught in hazards when the employer/on-site competent person failed to ensure a means of egress was provided while employees were in the excavation.  The excavation measured 9'6" wide by 5'4" deep by 41'9" long.
