The employee was installing siding from a pump jack scaffold and not protected against falls by personal fall protection or a complete netting system. Observed on or about 04/17/25.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Beardsley Ave. Long Beach, NJ: Employees were exposed to a structure collapse from a Alum-A-Pole pump jack scaffold. Employees were working at a height of approximately thirty feet from a pump jack scaffold that was improperly assembled. The jack was missing the required bottom braces. Observed on or about 04/17/25  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
