29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) New single-family residential structure located on Lot #149 at 134 Hensley Park Lane Dawsonville, GA 30534, Ground and scaffold: On or about 04/19/2025, the employer exposed employees to struck-by hazards while operating circular saws and pneumatic nail guns in that protective eyewear was not provided while cutting and installing siding material.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   a) New single-family residential structure located on Lot #149 at 134 Hensley Park Lane Dawsonville, GA 30534,  Scaffold area: On or about 04/19/2025, employee was exposed to fall hazards of approximately 17 feet 9 inches  to the lower ground below while utilizing a scaffolding system, in that a fall protection system was not available or utilized.

29 CFR  1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section.  a) New single-family residential structure located on Lot #149 at 134 Hensley Park Lane Dawsonville, GA 30534,  Ground: On or about 04/19/2025,and at time prior, the employer exposed employees to respiratory hazards due to silica exposure from cutting cement fiber siding board, in that engineering controls, work practices, or respiratory protection as described in Table-1 were not adhered to.
