29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) shall be done in the following sequence (as outlined by 29 CFR 1910.147(d)(1)-(6)): On or about April 15, 2025, hazardous energy was not managed through lockout or tagout during unjamming operations on the FORMAX Perforator-Cuber located on Line 6 in Processing Room 2.

29 CFR 1910.212(a)(3)(ii):Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On and before April 15, 2025, the interlocked point of operation guard on the FORMAX perforator-cuber, located in process room 2, line 6 (table pork loin line) was not designed and constructed to prevent employee contact with the point of operation knives of the perforator-cuber. The interlocked guard allowed for a large enough opening, prior to initiating a safe stop command, stopping the hazardous motion, before allowing employee access to the point of operation.
