OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized struck-by hazards that were causing or likely to cause death or serious physical harm to employees. a) Jobsite located at or near 3500 Civic Center Blvd, Philadelphia, Pennsylvania, on or about April 18, 2025 - Loading Dock: The employer did not ensure employees were not exposed employees to vehicles backing when employees are involved in activities related to positioning a lift gate to line up with a loading dock, thereby exposing employees to struck-by / caught between hazards.
