29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a. On or about April 21,  2025, at a private residence: employees were exposed to 8 foot 2 inch fall hazards while installing terracotta roof tiles on a 5:12 pitched roof, in that a fall protection system was not used.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a. On or about April 21,  2025, at a private residence: employees were exposed to 8 feet 2 inch fall hazard, in that a extension ladder was used and did not extend 3-feet above the roof landing to access a 5:12 pitch residential roof.  b.  On or about April 21,  2025, at a private residence: employees were exposed to fall hazard, in that a extension ladder was used and did not extend 3-feet above the second floor roof landing to access a 5:12 pitch residential roof.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a. On or about April 21,  2025, at a private residence: the employer did not prepare and maintain a written fall protection training program certification for employees exposed to fall hazards of 8 feet 2 inches while working on the roof.  Calix Contractors Inc  was previously cited for a violation of this occupational safety and health standard or its equivalent standard1926.503(b)(1), which was contained in OSHA inspection number 1787993, citation number 2, item number 2 and was affirmed as a final order on 03-25-2025, with respect to a workplace located at 1010 Lido Dr. Howey the Hills FL 34797.
