29 CFR 1910.333(c)(3): Where overhead lines were to be deenergized, arrangements had not been made with the person or organization that operated or controlled the electric circuits involved to deenergize and ground them:    a) 10 Chelwynne Rd., New Castle, DE 19720 (Rear Yard): On or about April 21, 2025, employees were exposed to electrical shock hazards while working in close proximity to the primary electrical lines while engaged in tree removal, where the electrical lines were not deenergized or insulated to prevent the hazard.

29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she could:    a) 10 Chelwynne Rd., New Castle, DE 19720 (Rear Yard): On or about April 21, 2025, the employer exposed employees to fall hazards from cutting a tree branch approximately 30'4" above ground without providing the employee with all protection training.
