29 CFR 1910.140(c)(22): The employer did not ensure that personal fall protection systems are worn with the attachment point of the body harness located in the center of the employee's back near shoulder level. The attachment point may be located in the pre-sternal position if the free fall distance is limited to 2 feet (0.6 m) or less. (a) On or about April 21, 2025, the employer failed to assure that employees operating a part picker from a height of approximately 20 ft., were using their lanyard with harness the correct way. Instead of the d-ring being centered between the shoulder, employees attached their lanyard to the d-ring(s) located at the hip of the harness exposing them to a fall hazard.
