29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 21, 2025, at the above jobsite, an employee was not wearing protective helmets where there was a possible danger of head injury from falling objects while performing roofing work.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a. On or about April 21, 2025, at the above addressed jobsite, an employee performing residential roofing operations, eight (10') feet three (3") inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  b. On or about April 21, 2025, at the above addressed jobsite,  employees performing residential roofing operations, sixteen (16') feet nine (9") inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.
