29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started. a) On or around April 21, 2025, on the east side of the facility, near the green end, employees were exposed to unguarded, energized overhead power lines while using a metal pole to clear wood debris from a surge bin machine located on an elevated work platform.

29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she could contact came closer than 10 feet. a) On or about April 21, 2025, on the east side of the facility, near the green end; employees failed to maintain a safe distance from energized overhead power lines while using a conductive metal pole to clear wood debris from a surge bin machine on an elevated work platform.
