29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Excavation located at Dollar Circle and Buford Hwy. NE, Suwannee, GA 30024: On or about 04/22/2025, employees were exposed to cave-in and engulfment hazards while working inside a trench measuring approximately 6 feet 4 inches in depth, 9 feet 6 inches in width, and 16 feet in length. The employer failed to provide a safe means of egress within 25 feet of lateral travel for employees performing replacement work on cast iron water piping.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) Excavation located at Dollar Circle and Buford Hwy. NE, Suwannee, GA 30024: On or about 04/22/2025, employees were exposed to cave-in and engulfment hazards while working inside an excavation measuring approximately 6 feet, 4 inches in depth, 9 feet, 6 inches in width, and 16 feet in length. The employer failed to provide an adequate protective system, such as shoring, shielding, or sloping, for employees performing water pipe and connection replacement work. The Dickerson Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1632768, citation number 1, item number 2 and was affirmed as a final order on 02-10-2023, with respect to a workplace located at 340 Amberwood Drive, Lawrenceville, GA 30044.
