29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Abner Vasquez dba ADJ Construction LLC on or about April 22, 2025, at the address of 519 Cranberry West, Findlay, OH: The employer failed to ensure the side rails of both extension ladders used to access the work area were extended at least three feet above the landing/working level. Employees utilizing the ladder in this condition were exposed to falls of approximately 10 ft. while using the ladder(s) to access and egress to and from the working level.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Abner Vasquez dba ADJ Construction LLC on or about April 22, 2025, at the address of 519 Cranberry West, Findlay, OH: The employer did not ensure employees were protected from falls while installing new roofing material to a residential type of structure. Employees operating in this condition were exposed up to an 18 ft. fall to the ground below. Abner Vasquez DBA ADJ Construction LLC was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.501(b)(11), which was contained in OSHA inspection number 1677173, citation number 1, item number 1a and was affirmed as a final order on September 11, 2023, with respect to a workplace located at 204 High St. McComb, OH.
