29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about April 22, 2025, the employer did not ensure that all employees using pneumatic nailers, which exposes employees to struck by hazards, were provided appropriate eye or face protection.

29 CFR  1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:    On or about April 22, 2025, the employer did not ensure that a scaffold platform that was being used was not properly secured to the mast of the all terrain fork lift.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about April 22, 2025, the employer did not ensure that employees being lifted with an all terrain fork lift were not protected to from falling to a lower level.
