29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: (a) 17731 County Road 2, Lot 1, Lochbuie, CO 80603: On or about January 21, 2025, the employer did not ensure that employees were trained to safely operate a Kubota Compact Track Loader (CTL) Model: SVL97-2. An employee was attempting to start the skid steer in below freezing temperatures. The employee reached into the engine compartment while the engine was sputtering and contacted moving parts which resulted in two of his fingers being amputated. This condition exposed employees to amputation hazards.
