Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. On or about April 23, 2025, employee(s) engaged in bakery activities on the Roll Line #2 located at 10608 Gertrude Street, Lavista, Nebraska. Employee(s) were exposed to amputation hazards associated with inadequately guarding in that the provided guard on Roll Line #2 Onion Belt, Feed Conveyor was not affixed in such a manner as to prevent the employee(s) from being in the danger zone during machine operations.

Employee(s) are exposed to amputation hazards. This was documented on April 23, 2025, at 10608 Gertrude Street, LaVista, Nebraska. Employee(s) were engaged in production operations that exposed them to unguarded V-belt drives and sprockets on the R27 attached to Roll Line #2.
