29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated (a) Jobsite at 7414 Janes Avenue, Woodridge, IL 60517 - On or about April 23, 2025, employees were installing a 20-foot lighted sign from the basket of an Altec TA41M truck mounted articulating boom lift, SN0300CL085, while at a height of thirteen (13) feet and were not wearing an approved body belt or other approved fall arrest harness system, thereby exposing them to a fall that can result in serious injury up to death. Abatement documentation is not required of this item.

29 CFR 1926.416(a)(1):The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work: (a) Jobsite at 7414 Janes Avenue, Woodridge, IL 60517 - On or about April 23, 2025, the employer did not ensure that an employee was protected against electrical contact when testing an unguarded overhead power line for voltage thereby exposing them to electrical hazards that may result in electrocution and a fall from height of 16 feet that may result in death. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
