29 CFR 1926.416(a)(3): The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take. On the 3rd Floor Deck in Area C: On or about April 1, 2025, employees were applying fire proofing to steel beams that were in close proximity to live powerlines without warning signs being posted.

On the 3rd Floor Deck in Area C: On or about April 1, 2025, a JLG 460SJ Telescopic Boom Lift/Mobile Elevated Work Platform was being operated within 10-feet of overhead powerlines carrying a voltage of 7200 volts.
