29 CFR  1910.178(l)(3)(i)(M): The employer did not review the operating instructions, warnings, or precautions listed in the operator's manual for the types of vehicle that the employee is being trained to operate:   a) On or about 04/17/2025, at the site located at 9778 Creek Road in Batavia NY; Employees were driving powered industrial vehicles (PIVs), such as, but not limited to: Vertical mast skid steer loader/rough terrain forklift truck (Kubota SSV75 with a vertical lift path being utilized with forks attachments). The employer did not review manufacturer operator's manuals with PIV operators.  An employee operating the  Kubota SSV75 vehicle was exposed to struck by hazards when forks on vehicle detached, swung downward into the operator's cab and injured employee's legs severely.  ABATEMENT DOCUMENTATION REQUIRED
