29 CFR 1910.94(b)(3)(i): Hoods connected to exhaust systems shall be used, and such hoods shall be designed, located, and placed so that the dust or dirt particles shall fall or be projected into the hoods in the direction of the air flow. No wheels, discs, straps, or belts shall be operated in such manner and in such direction as to cause the dust and dirt particles to be thrown into the operator's breathing zone. At an establishment located at 565 Olde Midway Road, Menasha, Wisconsin, in the Polishing Room, employees were exposed to inhalation hazards from dust and dirt particles created by the rotating buffing and polishing wheels in the following instances; a. On or about April 23, 2025, on the G&P Machinery Polishing Lathe along the northeastern wall of the Polishing Room, the hoods used were not connected to an exhaust system; b. On or about April 23, 2025, on the Hammond Variable Speed Polishing & Buffing Lathe along the eastern wall of the Plating Room, the hoods used were not connected to an exhaust system; c. On or about April 23, 2025, on the G&P Machinery Spindle Stand along the western wall of the Plating Room, hoods were not used; d. On or about April 23, 2025, on the G&P Machinery Spindle Stand along the northwestern wall of the Plating Room, a hood was not used on the right polishing wheel; e. On or about April 23, 2025, on the G&P Machinery Spindle Stand along the northwestern wall of the Plating Room, a hood was not connected to an exhaust system on the left polishing wheel

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are - barrier guards, two-hand tripping devices, electronic safety devices, etc. At an establishment located at 565 Olde Midway Road, Menasha, Wisconsin, in the Polishing Room, employees were exposed to caught-in hazards from in-going nip points created by the abrasive belt and the rotating wheels in the following instances; a. On or about April 23, 2025, on the Hammond UBG-132 Abrasive Belt Grinder, S/N 3854, along the southern wall of the Polishing Room, the abrasive belt and the rotating wheels were not guarded to prevent exposure to these hazards. b. On or about April 23, 2025, on the Hammond UBG-132 Abrasive Belt Grinder, S/N 9959, along the southern wall of the Polishing Room, the abrasive belt and the rotating wheels were not guarded to prevent exposure to these hazards. c. On or about April 23, 2025, on the Stephen Bader Co. Inc. BA-29 Abrasive Belt Grinder along the southern wall of the Polishing Room, the abrasive belt and the rotating wheels were not guarded to prevent exposure to these hazards. d. On or about April 23, 2025, on the Stephen Bader Co. Inc. On or about April 23, 2025, at an establishment located at 565 Olde Midway Road, Menasha, Wisconsin, in the Polishing Room, the Hammond UBG-132 Abrasive Belt Grinder, S/N 3854, had a threaded, projecting shaft which was not smooth or guarded by a nonrotating cap or safety sleeve exposing employees to entanglement hazards. b. On or about April 23, 2025, at an establishment located at 565 Olde Midway Road, Menasha, Wisconsin, in the Plating Room, the rotary tumbler along the western wall of the Plating Room, had a projecting end more than one-half the diameter of the shaft and unguarded by a non-rotating cap exposing employees to entanglement hazards.

On or about April 23, 2025, at the establishment located at 565 Olde Midway Road, Menasha, Wisconsin, in the Plating Room, the employer did not ensure that each container of hazardous chemicals was labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, exposing employees to the hazards associated with hazardous chemicals. a. A black plastic garbage can with a black to containing Hydrochloric Acid had no labeling to provide general information of the product contained inside. b. A rust-colored steel drum containing Zincate 16-70 had no labeling to provide general information of the product contained inside. c. A tall brown polymer container with an over-sized lid containing Nitric Acid had no labeling to provide general information of the product contained inside.

In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. On or about April 23, 2025, at the establishment located at 565 Olde Midway Road, Menasha, Wisconsin, employees were exposed to electrical and fire hazards from five outlet boxes missing covers along the southern and western walls of the Polishing Room.
