29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) On or about April 23, 2025, at a worksite located at EYA Townhomes Reservoir District 61 Service Road Washington DC; 2nd and 3rd floor, sub-contractor employees were exposed to an electrical hazard while using an extension cord (SJTW 12ga) that the strain had pulled away exposing the conductors.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about April 23, 2025, at a worksite located at  EYA Townhomes Reservoir District 61 Service Road Washington DC; 2nd and 3rd floor, sub-contractor employees conducting carpentry work did not have fall protection in place to prevent them from falling an estimated 30-feet while installing the trusses or while framing.  b) On or about April 23, 2025, at a worksite located at EYA Townhomes Reservoir District 61 Service Road Washington DC; interior 2nd floor, sub-contractor employees were exposed to estimated 10-foot falls while working near unguarded floor openings.  c) On or about April 23, 2025, at a worksite located at EYA Townhomes Reservoir District 61 Service Road Washington DC; interior 2nd floor, sub-contractor employees were exposed to estimated 10-foot falls while working near unguarded wall openings.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about April 23, 2025, at a worksite located at EYA Townhomes Reservoir District 61 Service Road Washington DC; 2nd and 3rd floor, the  sub-contractor workers framing the area were using a Werner 5-foot ladder to reach the deck where the rail was 5-feet lower than the work area (top plate). This exposed them to the risk of falling while using the ladder through the wall openings, estimated fall of 27-feet.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

This exposed them to the risk of falling while using the top step.
