29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about April 22, 2025, during the installation of roofing material at the job site located at Building #40, Parkview @ Springhill, Akron Ohio 44312; the employer did not ensure employees working on a roof of a residential structure were protected from a fall greater than 25 feet above the ground. The supervisor was on site and working along side the exposed employees.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 22, 2025, during the installation of roofing material at the job site located at Building #40, Parkview @ Springhill, Akron Ohio 44312; the employer did not ensure that portable ladders used to gain access/egress from the roof were extended 3 feet above the upper landing surface. The foreman was on site and utilized the portable ladder to gain access/egress to the roof.
