29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, were not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:  a) Job site, 313 East 5th Avenue, Tarentum, PA: On or about April 24, 2025 during roofing activities, the employer did not ensure appropriate fall protection was utilized in that existing parapet walls were determined to be approximately 30 inches in height, exposing employees to fall hazards of approximately 20 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that enabled each employee to recognize the hazards of falling and that trained each employee in the procedures to be followed in order to minimize these hazards:  a) Job site, 313 East 5th Avenue, Tarentum, PA: On or about April 24, 2025 during roofing activities, the employer did not ensure employees were provided appropriate fall protection training where a fall hazard of up to at least approximately 20 feet existed.
