29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Location: 34 Pine Street Holbrook, MA.   On April 25, 2025, employees were exposed to falls from heights up to 10 feet to the ground while accessing an upper landing surface where an extension ladder was used to access the roof where the ladder side rails did not extend at least 3 feet above the edge of the roof.

29 CFR  1926.1053(b)(10):The tops of a non-self-supporting ladder with more than one support attachment were not placed with the two rails supported equally:  Location: 34 Pine Street Holbrook, MA.   On April 25, 2025, employees were exposed to falls from heights up to 10 feet to the ground while accessing an upper landing surface where an extension ladder was used to access a residential roof, the ladder was set up on the gable end of the roof, and the side rails of the ladder were not supported equally.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  Location 34 Pine Street Holbrook, MA.  On or about  April 25, 2025, the employer did not ensure that eye protective equipment was used when employees were exposed to eye injury while using pneumatic nail guns to install shingles and ply wood roofing materials  The Jhuniors Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1687475, citation number 1, item number 1 and was affirmed as a final order on October 20, 2023, with respect to a workplace located at 179 Second Street East Providence, RI 02914.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501   Location 34 Pine Street Holbrook MA.  On or about April 25, 2025, Employees were exposed to fall hazards while working on a roof approximately 10 feet off the ground with out any fall protection measures in place.
