29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  a) Top plate: On or about April 25, 2025, the employer exposed employees to fall hazards of approximately 9-feet to the lower 2nd floor level, in that a ladder or other safe means of access was not provided to access/egress the top plate while engaged in framing activities. Employees were required to climb the framed walls to reach the working level.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a) Ceiling Joists, On or about April 25, 2025, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns, in that protective eyewear was not provided to employees engaged in framing activities while operating these tools at 150 PSI.  Reynaldo Geovanny Reyes Calero was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1726955, citation number 1, item number 1, and was affirmed as a final order on  05/14/2024, with respect to a workplace located at 7415 Barclay Court (Lot# 2 Addison Grove Subdivision) Dawsonville, GA 30534.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a) Top plate: On or about April 25, 2025, and at times prior, the employer exposed employees to fall hazards of approximately 30 feet to the lower level, in that a fall protection system was not provided to employees engaged in framing activities.  b) Second level: On or about April 25, 2025, the employer exposed employees to fall hazards of approximately 12 feet to the lower level, in that a fall protection system was not provided to employees engaged in framing while working with an unprotected side or edge.  Reynaldo Geovanny Reyes Calero was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1726955, citation number 1, item number 2, and was affirmed as a final order on  05/14/2024, with respect to a workplace located at 7415 Barclay Court (Lot# 2 Addison Grove Subdivision) Dawsonville, GA 30534.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) On or about April 25, 2025, the employer failed to inspect all electrical extension cords used to provide power to electric tools which employees engaged in framing operations were using, in that the yellow extension cords powering the reciprocating, circular saw, impact drills, and battery chargers, were damaged. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.  Reynaldo Geovanny Reyes Calero was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.403(b)(1), which was contained in OSHA inspection number 1726955, citation number 2, item number 1, and was affirmed as a final order on  05/14/2024, with respect to a workplace located at 7415 Barclay Court (Lot# 2 Addison Grove Subdivision) Dawsonville, GA 30534.
