29 CFR  1926.20(b)(2):Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.   a) Located at Building 3-5011 Silverdale Drive, Canal Winchester, OH 43110: On or about April 25, 2025, the employer did not ensure safety and health program(s) were developed to include frequent and regular inspections of the job site. As a result, employees were exposed to fall hazards greater than 30 feet while framing and sheathing residential structure.

29 CFR  1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:   a) Located at Building 3-5011 Silverdale Drive, Canal Winchester, OH 43110: On or about April 25, 2025, employees were performing residential construction without the use of proper anchorage with ropes wrapped around the trusses and tied back to itself, thereby exposing employees to fall hazards of more than 30 feet.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a) Located at Building 3-5011 Silverdale Drive, Canal Winchester, OH 43110: On or about April 25, 2025, the employer did not ensure that employees on the job site were protected from falling by a fall protection system. As a result, employees were exposed to a fall hazard of greater than 30 feet while performing residential construction.  Leonel's Framing LLC was previously cited under the name Leonel Garcia DBA Leonels Framing LLC for a violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1622596, citation number 1, item number 1 and was affirmed as a final order on 12/27/2022, with respect to a workplace located at 1341 Rapp Street, Lewis Center, OH 43035.

29 CFR  1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   a) Located at Building 3-5011 Silverdale Drive, Canal Winchester, OH 43110: On or about April 25, 2025, the employer failed to ensure that each employee exposed to a fall hazard was trained on the hazards of falling and the procedures to minimize the hazards in the work area. As a result, employees were exposed to fall hazards greater than 30 feet while performing residential construction.  Leonel's Framing LLC was previously cited under the name Leonel Garcia DBA Leonels Framing LLC for a violation of this Occupational Safety and Health standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1622596, citation number 1, item number 2 and was affirmed as a final order on 12/27/2022, with respect to a workplace located at 1341 Rapp Street, Lewis Center, OH 43035.
