29 CFR 1926.102(a)(1) : The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles. (a) On or about April 24, 2025, at 1040 Oconomowoc Parkway, Oconomowoc, WI 53066, the employer did not ensure employees conducting roofing operations, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazards and eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about April 24, 2025, at 1040 Oconomowoc Parkway, Oconomowoc, WI 53066, an employee installing a roof on a new residential condominium with a 4:12 pitch was not using fall protection. This exposed the employee to a fall hazard of approximately 23 feet 3 inches from the roof eve to the ground below on the front (south) side of the complex.
