29 CFR 1926.502(d)(9): Lanyards and/or vertical lifelines did not have a minimum breaking strength of 5,000 pounds (22.2 kN). Location: Roof On or about 4/29/2025, employees were exposed to fall hazards greater than 20 feet to the ground below when using a personal fall arrest system consisting of ropes for vertical lifelines that were damaged, knotted, and/or abraded, reducing their minimum breaking strength to less than 5,000 pounds.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee. Location: Roof On or about 4/29/2025, employees were exposed to fall hazards greater than 20 feet to the ground below when using a personal fall arrest system consisting of a Miller RA25-1 reusable roof anchor that was improperly installed, in that, only a total of two 16d duplex nails (two nails per metal flap) were used to secure the ring to the ridge beam. According to the manufacturer, the minimum number of fasteners required to achieve the 5,000 pound anchor strength is a total of twelve (12) new 16d nails OR twelve (12) new #8 2-?" (63.5mm) or longer screws (six on each side) secured into a wood support member through the middle row of holes on the anchor.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. Location: Roof On or about 4/29/2025, employees were exposed to head injuries while working unprotected on the ground below the roof, where the employer failed to provide them with hardhats, and while other employees were performing roofing activities at a height of an estimated 20 feet above. JQV Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1760696, citation number 1, item number 1 and was affirmed as a final order on 10/22/2024, with respect to a workplace located at 37 Cardinal Road, Cranston, RI 02921.
