On April 23, 2025 an employee fell approximately 11 feet from a ladder jack scaffold where the scaffold was not erected under the supervision of a competent person.

29 CFR 1926.452(k)(3): The ladder jack shall be so designed and constructed that it will bear on the side rails and ladder rungs or on the ladder rungs alone. If bearing on rungs only, the bearing area shall include a length of at least 10 inches (25.4 cm) on each rung Location 3 North Main Street Cohasset MA. (A) The ladder jack was not so designed and constructed as required. On or about April 23, 2025, employee was exposed to fall hazard where a ladder jack scaffold was not properly set up and the plank ran through a rung of a ladder instead of on the jack itself.

On April 23, 2025 employees were exposed to fall hazards where an extension ladder to gain access to a ladder jack scaffold plank was not extended at least 3 feet above the landing surface.
