29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: 400 Whitney Ave., Holyoke, MA: On or about April 29, 2025, the second floor of the store was not kept in clean, orderly and sanitary condition due to the presence of multiple water-logged, damaged and collapsed ceiling tiles, and stained/dirty columns and shelves, in work areas in the fulfillment department, due to water which had previously leaked from the ceiling.
