29 CFR  1926.95(d)(1):Except as provided by paragraphs (d)(2) through (d)(6) of this section, the protective equipment, including personal protective equipment (PPE), used to comply with this part, was not provided by the employer at no cost to employee(s).  a. On or about April 30, 2025, at the site: the employer did not provide the necessary PPE at no cost to the employees (fall, eye and head protection) while conducting framing activities, exposing employees to falls hazards. Employees purchased their own personal protective equipment, and it was not maintained by the employer.

On or about April 30, 2025, at the site: employees were exposed to approximate 18 foot fall hazards while changing the decking and installing shingles on a 8:12 pitched roof, in that a fall protection system was not used.  a. On or about April 30, 2025, at the site: employees were exposed to approximate 9 foot fall hazards while changing the decking and installing shingles on a 4:12 pitched roof, in that a fall protection system was not used.
