29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a.)  Warehouse - On or about April 30, 2025, the employer failed to ensure an energy control program consisting of an energy control procedure, employee training and periodic inspections was established for employees performing service and maintenance on machines including, but not limited to the Stamtec OPC-200 power press,  Armorcraft punch press, Leland-Gifford drill press, Wilton A2608 drill press, and Verson press brake machines, thereby exposing employees to hazards associated with rotating and moving parts.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.)  Warehouse - On or about April 30, 2025, the employer failed to provide effective guarding on the Leland-Gifford Model 1 LMS 2518 and Wilton A2608 drill press machines, thereby exposing an employee to crush hazards, broken bones, and amputations as the employee is required to use the machines daily to make parts, including but not limited to, gear shafts.

29 CFR  1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program that included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   a.) Warehouse - On or about April 30, 2025, the employer did not develop and implement a written hazard communication program in accordance with 29 CFR 1910.1200(e)(1) that would describe or include at least the following:  1) Requirement for labeling of containers of hazardous chemicals; 2) Safety data sheet availability; 3) Training of employees; 4) A complete list of hazardous chemicals known to be present in the workplace; 5) Methods to inform employees of the hazards on non-routine tasks; and 6) Methods to inform other employer(s) of material safety data sheet availability; the labeling system and any precautionary measures to protect employees.  Employees are required to work with, or around, highly flammable and toxic chemicals, including but not limited to Klean Strip, Toluene, and Xylene.

29 CFR 1910.1200(h)(1):  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a.) Warehouse - On or about  April 21, 2025, employees used toluene to remove blemishes from finished parts.  The employer did not ensure that  employees were trained in the hazards associated with chemicals such as, but not limited to, toluene, xylene, and Klean Strip.
