Evans General Contractors, LLC, located at 3050 Northwinds Parkway, Suite 200, in Alpharetta, GA 30009: On or about April 30, 2025, the employer failed for calendar year 2024, to electronically submit information from the OSHA Form 300A or equivalent by March 2, 2025.
