On top of a structure located at 25 Catalonia Court, Building 24, St. Augustine Beach, FL: On or about April 30, 2025, the employer exposed employees to fall hazards, in that the 20-Foot Werner extension ladder used for access was not extended at least three feet above the upper landing surface to provide hand hold points to employees accessing a 17-foot 8-inch roof for dryer ventilation duct maintenance.
