29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about April 30, 2025, the employer failed to develop machine specific lockout tagout procedures for the Makino-a71 CNC machine. Maintenance personnel engaged in covered activities (i.e., repairs, adjustments) and exposed to moving parts from unexpected re-energization of equipment. b) On or about April 29, 2025, the employer failed to develop machine specific lockout tagout procedures for the Danly Power Press #119 to protect operators from the unexpected re-energization. Operators engaged in covered activities (i.e., setup, adjustments, change outs) and were exposed to crushing and amputation hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about April 24, 2025, in the tooling area, Whacheon lathe (S/N: 9-8110-27) operators were not protected against rotating parts (i.e., chuck) during cutting/drilling activities. Lathe operators' hands/arms were exposed to be pulled into the chuck exposing employees to a crushing and amputation hazard. b) On or about and times prior to April 30, 2025, in the tooling area, Doosan Mynx Model 7500/50 CNC machine operators were not protected against rotating parts and flying chips during machine cycling. The Doosan CNC machine's interlock was defeated exposing employees to struck by and flying chip hazards made from the spinning tool bit.
