(a) Pallet production building: On or about April 30, 2025, the employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces were kept in a clean, orderly, and sanitary condition in that the overhead conduits, lighting fixtures, and horizontal surfaces had accumulations of dusts from the wood pallet processing operations in the following areas. 1) Bad pallet room: Accumulations of wood debris on the floor underneath stacks of pallets and processing table. 2) Pallett repair Area: Accumulations of wood debris on the floor underneath stacks of pallets.
