29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  Location: 1636 East 51st Street, Brooklyn, NY.  On or about 4/29/2025,  a) Two employees working from the 1st floor roof did not utilize any fall protection.

Location: 1636 East 51st Street, Brooklyn, NY.  On or about 4/29/2025,  a) Two employees working atop the 1st floor roof installing joists for the roof and did not recognize the hazards associated with hazards of working next to an unprotected edges of the roof.

Location: 1636 East 51st Street, Brooklyn, NY.  On or about 4/29/2025  a) The extension ladder used by employees to access the roof was extend to approximately 1 foot above the roof level.
