(a) At 607 Northwest 27th Avenue, Ocala, FL; On or about April 30, 2025: the employer exposed employees to trip, fall and ejection hazards, in that the walking-working surface in the warehouse was not maintained free of potholes. A pothole in the warehouse area measured 13" long, 26" wide, and 2 1/2" deep.
